Notice of Electronic Disclosure of Protected Health Information

Your Privacy & Electronic Records To provide you with the most convenient and flexible care, my practice uses a secure electronic system to manage and store your health records. Because I use these digital tools, Texas law requires me to let you know that your health information is subject to "electronic disclosure."

What this means for you:

  • Routine Care: I share your information electronically only for standard reasons like coordinating your treatment, processing payments, or as required by law.

  • Your Permission: Your story belongs to you. If you ever need a copy of your health records, the quickest way is to complete an Authorization to Release Records Request form on my contact page. Once I receive your request, I will send you an Authorization form through simple practice within 24 hours. I will provide your digital records to you within 15 business days of receiving your signed authorization. Since we work together virtually, I’ll ensure these are sent through a secure, encrypted link to keep your privacy protected.
